Instrument Rental Agreement

Dear Parent:

We are pleased to provide you or your child with a school-owned instrument for lessons with us. A rental fee of $40.00 is charged monthly for the maintenance of our school-owned instruments.

Kindly fill out this rental agreement. You only need to fill out the boxes. We already have your credit card information that you gave us in your registration and you will see a $40 charge at the beginning of each month.
